Rebuilding Program
The Rebuilding Program serves adults brain injury survivors who have experienced a change in their cognitive functioning (thinking skills) following brain injury. These changes to thinking skills may pose barriers to participating in everyday activities and being involved in the community. The Rebuilding Program offers one-on-one community rehabilitation services to enable participants to relearn cognitive skills and improve independent functioning in the community.
Program participants are matched with a Rehabilitation Facilitator to develop and work towards goals together. Depending on the goals set, the participant and Rehabilitation Facilitator may meet in-office, in-home, or at various sites in the community on a weekly basis. The outcomes of the program are changes in satisfaction and performance of everyday activities as measured by standardized assessments. The Rebuilding Program aims to work closely with family and other service providers to maximize participant success.

Cognitive Skill Development
Our rehabilitation professionals work with participants to identify cognitive concerns. These concerns are addressed through a systematic process involving the development of strategies, transfer of strategies into real-life situations, and the retraining of cognitive skills.
Cognitive strategies
- Modification of the way activities are carried out
- Modification of the environment in which everyday activities occur
Retraining of cognitive skills
- Cognitive retraining activities and exercises completed in-office or in the home
- Brain Fitness computer program

Personal Life Management
- Participants relearn how to manage daily challenges. Practical strategies are taught to enable greater independence and participation in daily life.
- Assessments and inventories may be used to identify areas of need.
- Fatigue management
- Maintaining physical health and fitness
- Managing transportation
- Household management


Productive Activity
Participants are assisted in re-engaging in meaningful roles in the community. Assessments and inventories are available to the participant to help identify strengths and areas of interest.
- Accessing and maintaining volunteer opportunities
- Accessing leisure opportunities
- Accessing education opportunities
- Preparation for employment
Program Length
The Rebuilding Program offers 10 months of Direct Service where the participant and Rehabilitation Facilitator meet weekly. Following Direct Services, participants can access a 2- year Follow-Along period. During the Follow-Along period, participants are welcome to request re-initiation of weekly services to revisit goals developed during the initial 10 months.
Program Admission Criteria:
The Rebuilding Program is best suited for individuals who:
- Have sustained a moderate to severe brain injury after the age of 16
- Are currently 18 years of age or older
- Are independent with physical needs OR have assistance in this area
- Are capable of meaningful communication
- Do not have a progressive medical condition
- Are managing issues such as substance abuse, mental illness and/or chronic pain
- Are ready to take responsibility for learning new skills and strategies
- Are currently living in the Ebirs
